In order for us to register your child for camp, we MUST receive the following:
- Completed Application
- Emergency Form
- A copy of your child's most recent physical *
(dated within 24 months of attending camp) - A copy of your child's immunization record *
- Full Payment (New! Pay online with any major credit card via Pay Pal!)
* Even if your child attended camp last summer – we still need a NEW copy of your child’s physical/immunization record. We do not have the office space to archive physicals from previous summers.
